It should work if you set the layout property to "GridLayout":

<f:SimpleForm columnsL="1" editable="true" emptySpanL="4" emptySpanM="4" id="NewRequestform" labelSpanL="3" labelSpanM="3"  layout="GridLayout" minWidth="1024">

The default is ResponsiveGridLayout and this results in arranging labels and input fields one below the other on phone devices (as you observed).
