If something happens only in production but not in Development or Quality systems then this usually means that an error got fixed but this fix is not yet transported to production.
Or it means you fixed it directly in production and missed the other systems (should not really happen, but I am not anymore surprised after reading SCN discussions for more than 10 years)
Or the data used is different from system to system.
